Citrus in Action: Huntington’s Disease

Learn from Mr. Hernandez's journey
Aug 15, 2024

It can take 5-7 years for people with Huntington’s disease to get an accurate diagnosis. We need faster testing, clearer diagnoses, and better care.

Citrus partners with rare disease patient advocacy groups and foundations to help patients and care team members alike search and identify optimal providers and organizations. In October 2024, we worked with patients connected to the Huntington’s Disease Society of America to route them to providers who were conveniently located, available, in-network, and cost-effective.

One such case involved Mr. Hernandez, a 38-year-old man from San Rafael, CA, who initially presented with severe mood swings, cognitive difficulties - including memory and learning issues - and notable apathy. His previous providers diagnosed him with schizophrenia and depression, prescribing a treatment regimen that included first-generation (typical) antipsychotics and cognitive behavioral therapy. While these treatments helped manage his psychiatric symptoms to some extent, within a year, Mr. Hernandez developed involuntary jerking movements in his hands and face.

His primary provider attributed these symptoms to dyskinesia, a common side effect of the antipsychotics. However, as the muscle spasms worsened, Mr. Hernandez grew increasingly concerned and sought additional medical opinions. Despite his efforts, finding an experienced specialist in his area proved difficult. Fortunately, he was referred to Citrus, where we discerned key patterns in his medical history and patient journey. We recognized the need for a more thorough evaluation, guiding Mr. Hernandez to a specialist who promptly diagnosed him with Huntington’s disease.

With the correct diagnosis in hand, Mr. Hernandez was switched to second-generation (atypical) antipsychotics, which more effectively managed his chorea and significantly reduced his movement-related episodes. Antidepressants were also prescribed, greatly improving his mood and quality of life. To ensure comprehensive care, Citrus collaborated with the recommended provider to assemble a multidisciplinary team, including a neurologist, psychiatrist, physical therapist, and nutritionist, ensuring that Mr. Hernandez’s immediate and long-term medical needs were fully addressed.
